White River Family Practice Receives Award of Excellence
White River Junction — White River Family Practice recently received the 2013 Ambulatory HIMSS Davies Award of Excellence. The award, presented by the Healthcare Information and Management Systems Society, recognizes excellence in health information technology, specifically in the use of electronic health records to improve quality of care and patient safety while achieving a return on investment.
White River Family Practice is an independent ambulatory care practice that provides care to approximately 12,000 patients, most of whom live in rural Vermont and New Hampshire. Under its electronic health record system, the practice has implemented EHR-enabled workflows and order sets and created an EHR-supported collaborative environment designated a “clinical microsystem,” described as a small group of people who regularly work together to provide care and the patients who receive it.
The system has allowed the practice to achieve “significant improvement in the quality of care for patients with chronic conditions like ischemic vascular disease, asthma and diabetes,” the society said. “The EHR was also leveraged to significantly improve smoking and alcohol cessation intervention with their patient population.”
The quality improvements resulted in White River Family Practice’s certification as a Level III Patient Centered Medical Home by the National Committee for Quality Assurance.
